Here's the simple version: coffee charcoal works like a sponge in your gut. Because of its porous, charcoal-like surface, it can grab onto medicines you swallow and keep them from being fully absorbed. With bosentan (Tracleer), that could mean less of your dose actually reaches your bloodstream, so it may not control your pulmonary arterial hypertension as well. Over time that might show up as more shortness of breath or trouble keeping up with activity.

This concern is based on how charcoal behaves rather than direct studies, so it's more theoretical than proven. Still, an easy fix is to space them out. Taking bosentan at least two hours apart from coffee charcoal helps. Please check with your pharmacist or doctor before combining them.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
