Here's the simple version. Coffee charcoal has a porous, sponge-like surface, much like activated charcoal. When it sits in your gut alongside a pill, it can grab onto the medicine and keep some of it from being absorbed. Your Premarin with Methyltestosterone is swallowed and taken up through the gut, so taking it too close to coffee charcoal could mean less of the hormone gets into your body.

In real life, that might show up as weaker symptom control, for example more hot flashes returning. This is a theoretical concern based on how charcoal behaves, not strong human studies, so don't panic. An easy fix is spacing them out by at least two hours. Run the timing past your pharmacist or doctor to be safe.
