Here's the simple version: coffee charcoal acts a bit like the activated charcoal you may have heard about. It has a porous surface that can grab onto things in your stomach and gut, including your saxagliptin (Onglyza) tablet. If they're sitting in your digestive tract at the same time, the charcoal may soak up some of the medication before your body can absorb it, so less of the drug actually gets into you.
For saxagliptin, that could mean it doesn't work as well at lowering your blood sugar after meals, and you might see higher readings than usual. The easy fix is timing: keeping these at least two hours apart usually avoids the problem. This concern is based on how charcoal behaves rather than firm studies, but it's a sensible step.
