Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: coffee charcoal acts a bit like activated charcoal in your gut. Its porous surface can grab onto medications you swallow, including selpercatinib (Retevmo), before your body has a chance to absorb them. If less of your cancer medicine gets into your bloodstream, it may not work as well as it should, and that matters a lot with a drug treating lung or thyroid cancer.

The good news is this is a theoretical concern based on how charcoal behaves, not strong human data, and it's easy to manage by spacing them out. The usual advice is to take them at least two hours apart. Please run this by your oncology pharmacist or doctor before combining them, so your selpercatinib keeps doing its job.

From our pharmacist’s overview on the Selpercatinib page

“People facing a RET-driven lung or thyroid cancer often add supplements for strength, sleep, or immune support, and with selpercatinib (Retevmo) that instinct needs a careful check. Eleven of the 178 ingredients here carry a major rating, and the largest cluster affects how the drug is cleared by the CYP3A4 enzyme: St.”

About Coffee Charcoal

What is Coffee Charcoal?

  • Diarrhea
  • Inflammatory bowel conditions
  • Irritable bowel syndrome (IBS)
  • Mild digestive upset
  • Wound and skin care (topical, traditional)

Coffee charcoal is made by roasting green coffee beans until they turn black, creating a charcoal-like material used traditionally for diarrhea and digestive complaints. Scientific evidence supporting its benefits is very limited, and it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a chronic gut condition.

About Selpercatinib

What Is Selpercatinib Used For?

Brand name: Retevmo

Selpercatinib treats cancers driven by changes in the RET gene — either a gene fusion (where RET joins abnormally with another gene) or a RET mutation (a change in the gene itself). Your doctor will confirm one of these changes with an FDA-approved test before prescribing it.

It is approved for adults with RET fusion-positive non-small cell lung cancer; adults and children 2 and older with RET-mutant medullary thyroid cancer or RET fusion-positive thyroid cancer; and adults and children 2 and older with other advanced solid tumors that have a RET gene fusion and have not responded to prior treatment.
