Belladonna Alkaloids, Phenobarbital falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS — the group Coltsfoot interacts with.

Hepatotoxic pyrrolizidine alkaloids (PA) are substrates of cytochrome P450 3A4 (CYP3A4). Theoretically, drugs that induce CYP3A4 might increase the conversion of PAs to toxic metabolites. Some drugs that induce CYP3A4 include carbamazepine (Tegretol), phenobarbital, phenytoin (Dilantin), rifampin, rifabutin (Mycobutin), and others.

Here's the concern with pairing coltsfoot and a product like Donnatal. The worry isn't really about your medication's effects, it's about the coltsfoot itself. Raw coltsfoot can contain pyrrolizidine alkaloids, which are compounds that can harm the liver. The phenobarbital in your medicine speeds up a liver enzyme called CYP3A4, and that same enzyme can turn those plant compounds into more toxic forms. So in theory, taking them together could nudge up the risk of liver injury from the herb.

I'll be honest with you: this is based on lab and animal data, not real patient studies, so it's more of a caution than a proven danger. Still, given coltsfoot's existing liver concerns, I'd choose a product certified free of pyrrolizidine alkaloids, or skip it. Please run it by your pharmacist or doctor before combining them.
