Due to its pyrrolizidine alkaloid constituents, comfrey can cause hepatotoxic effects, including ascites, cirrhosis, hepatic fibrosis, hepatomegaly, and sinusoidal obstruction syndrome.

Here's the concern with this combo. Comfrey contains natural compounds called pyrrolizidine alkaloids that can be hard on your liver, especially when taken by mouth. The product you asked about includes acetaminophen (the Tylenol portion), which your liver also has to process. Stacking two things that stress the same organ could, in theory, add up to more liver strain than either one alone.

To be clear, this is a theoretical caution based on how these substances work, not on documented cases with this exact pill. Still, comfrey really should never be swallowed. If you do use a comfrey cream on intact skin, the risk is much lower. Watch for yellowing skin or eyes, dark urine, or belly pain, and check with your pharmacist or doctor before combining them.
