Durvalumab falls under IMMUNOSUPPRESSANTS — the group Cordyceps interacts with.

Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.
Animal and in vitro research suggests that cordyceps stimulates the immune system. However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine, which suggests that cordyceps may have an immunosuppressive effect.

Here's the thing to keep in mind with Imfinzi (durvalumab): it's an immunotherapy drug. It works by taking the brakes off your immune system so it can attack the cancer. That's different from the immunosuppressants in our interaction file, which calm the immune system down. So the concern here is the opposite direction of fighting cancer.

Cordyceps may nudge the immune system in either direction (some lab data hints it stimulates it, while a small human study suggests it can act immunosuppressively). Because durvalumab depends on a properly working immune response, anything that unpredictably alters immune activity is worth a conversation. This is mostly theoretical and based on weak evidence, so don't panic, but please run cordyceps by your oncologist or pharmacist before adding it during treatment.

Cordyceps is a fungus used in traditional Chinese medicine for energy, exercise performance, and lung and immune support. Human research is limited and mostly low quality, so its benefits are not well proven. It appears generally well tolerated in the short term, but quality varies and it should be avoided in pregnancy and breastfeeding due to lack of safety data.

About Durvalumab Injection

What Is Durvalumab Injection Used For?

Brand name: Imfinzi

Imfinzi is used to treat a wide range of cancers in adults. For lung cancer, it is approved for resectable non-small cell lung cancer (given before and after surgery), unresectable Stage III non-small cell lung cancer (after chemoradiation), metastatic non-small cell lung cancer, limited-stage small cell lung cancer (after chemoradiation), and extensive-stage small cell lung cancer.

Beyond lung cancer, Imfinzi is also approved for locally advanced or metastatic biliary tract cancer (cancers of the bile ducts and gallbladder), unresectable hepatocellular carcinoma (liver cancer), mismatch repair deficient (dMMR) advanced or recurrent endometrial cancer, BCG-naive high-risk non-muscle-invasive bladder cancer, muscle-invasive bladder cancer, and resectable gastric or gastroesophageal junction adenocarcinoma (stomach cancer at the junction with the esophagus). Most of these uses involve Imfinzi given alongside other chemotherapy drugs.
