Hydrocortisone falls under IMMUNOSUPPRESSANTS — the group Cordyceps interacts with.

Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.
Animal and in vitro research suggests that cordyceps stimulates the immune system. However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine, which suggests that cordyceps may have an immunosuppressive effect.

Here's the gist with cordyceps and hydrocortisone. Hydrocortisone is a steroid that calms down an overactive immune system, so it's often used to settle inflammation, allergic reactions, or autoimmune flares. The concern is that cordyceps may nudge the immune system the opposite way, and that could theoretically tug against the very thing your steroid is trying to do.

Honestly, the evidence here is thin and mostly comes from animals, lab dishes, and one small study on a different drug. So I wouldn't lose sleep over it, but I also wouldn't ignore it, especially if your hydrocortisone is treating something serious. If you take both, watch for your underlying condition acting up, and please loop in your pharmacist or doctor before changing anything.

Cordyceps is a fungus used in traditional Chinese medicine for energy, exercise performance, and lung and immune support. Human research is limited and mostly low quality, so its benefits are not well proven. It appears generally well tolerated in the short term, but quality varies and it should be avoided in pregnancy and breastfeeding due to lack of safety data.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
