Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Cordyceps interacts with.

Theoretically, cordyceps may increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
In vitro and animal research suggests that cordyceps extract inhibits platelet aggregation and function. However, this interaction has not been reported in humans.

Here's the thing with iloprost (Ventavis): besides relaxing the blood vessels in your lungs, it also makes your platelets a little less sticky, so it gently nudges you toward easier bleeding on its own. Cordyceps may do something similar. In lab and animal studies, cordyceps extract slowed platelets from clumping together, so pairing the two could, in theory, add up to a higher bleeding risk.

I want to be honest about the evidence though: this has not actually been reported in people, and it comes only from test-tube and animal research. So I wouldn't panic. Just keep an eye out for things like easy bruising, nosebleeds, bleeding gums, or cuts that take longer to stop, and run the combination by your doctor or pharmacist before starting cordyceps.

Aurlumyn (iloprost injection, given intravenously) is approved to treat severe frostbite in adults, specifically to reduce the risk of losing fingers or toes (digit amputations). Its effectiveness was established in young, healthy adults who experienced frostbite at high altitudes.
