Omalizumab falls under IMMUNOSUPPRESSANTS — the group Cordyceps interacts with.

Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.
Animal and in vitro research suggests that cordyceps stimulates the immune system. However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine, which suggests that cordyceps may have an immunosuppressive effect.

Here's the thing to know about pairing cordyceps with Xolair (omalizumab): this is mostly a theoretical concern, not something well proven in people. Omalizumab works by binding and lowering IgE, the antibody that drives allergic asthma, nasal polyps, and chronic hives. Cordyceps, on the other hand, has shown mixed effects on the immune system in lab and animal studies, sometimes stimulating it and sometimes calming it.

Because cordyceps could in theory nudge your immune response in either direction, there's a chance it might slightly tug against how well Xolair controls your symptoms, like more wheezing or hives flaring back up. But the evidence here is weak. I wouldn't lose sleep over it. Just let your pharmacist or doctor know before adding a concentrated cordyceps supplement so they can keep an eye on your control.

Cordyceps is a fungus used in traditional Chinese medicine for energy, exercise performance, and lung and immune support. Human research is limited and mostly low quality, so its benefits are not well proven. It appears generally well tolerated in the short term, but quality varies and it should be avoided in pregnancy and breastfeeding due to lack of safety data.

About Omalizumab Injection

What Is Omalizumab Injection Used For?

Brand name: Xolair

Xolair is used for several allergy-driven conditions. It treats moderate to severe persistent asthma in adults and children 6 and older whose symptoms aren't controlled by inhaled corticosteroids alone and who test positive for a perennial allergen (like dust mites or pet dander). It also treats chronic rhinosinusitis with nasal polyps (ongoing sinus inflammation with polyp growth) in adults 18 and older when nasal corticosteroids haven't worked well enough.

Xolair is approved to help reduce allergic reactions — including anaphylaxis (severe, life-threatening allergic reactions) — from accidental food exposures in adults and children as young as 1 year old with IgE-mediated food allergy. It is always used alongside food allergen avoidance, not as a substitute. Finally, Xolair treats chronic spontaneous urticaria (CSU — ongoing hives with no known cause) in adults and adolescents 12 and older who still have symptoms despite taking antihistamines. It is not for emergency treatment of allergic reactions and not for other forms of hives.
