Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group Coriander interacts with.

Theoretically, coriander might increase the risk of hypotension when taken with antihypertensive drugs.

Opsynvi combines two blood pressure lowering medicines: macitentan (which relaxes blood vessels in the lungs) and tadalafil (the same active ingredient found in Cialis). Both can already nudge your blood pressure down. The concern here is simply additive: some animal studies hint that coriander may also lower blood pressure a little, so stacking it on top could, in theory, push things too low.

If that happened, you might feel lightheaded, dizzy, or faint, especially when standing up quickly. Keep in mind this is theoretical, based only on animal research, and the coriander (cilantro) you sprinkle on your tacos is very unlikely to cause trouble. Concentrated extracts or essential oils are a different story, so run those by your pharmacist or doctor first.
