Why this applies to Apalutamide

Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cubebs interacts with.

Several compounds isolated from cubebs can inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this effect has not been reported in humans. Theoretically, cubebs might increase the effects and side effects of drugs metabolized by CYP3A4. These include lovastatin (Mevacor), clarithromycin (Biaxin), cyclosporine (Neoral, Sandimmune), diltiazem (Cardizem), estrogens, indinavir (Crixivan), triazolam (Halcion), and numerous others.

Here's the gist for Erleada (apalutamide). Your body partly relies on a liver enzyme called CYP3A4 to clear this medication. In test-tube studies, certain compounds in cubeb berries appear to slow that same enzyme down. If that happened in people, apalutamide could build up a bit, which in theory might mean stronger effects and more side effects like fatigue, joint pain, rash, high blood pressure, or, rarely, seizures.

I want to be honest about how thin this evidence is, though. So far this has only been seen in the lab, never confirmed in humans, so it's mostly theoretical. A sprinkle of cubeb as a kitchen spice is very unlikely to matter. Concentrated cubeb extracts or oils are a different story, so check with your pharmacist or oncologist before using those.

About Apalutamide

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
