Aspirin, Codeine Phosphate falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Eleuthero interacts with.

Theoretically, eleuthero may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research shows that a constituent of eleuthero, dihydroxybenzoic acid, appears to inhibit platelet aggregation. Concomitant use with anticoagulant or antiplatelet drugs might increase the risk of bleeding. This effect has not been reported in humans.

Here's the thing to keep in mind with this combination: the aspirin part of your medication already thins the blood a bit by making your platelets less sticky. Eleuthero may do something similar. In test tube and animal studies, one of its compounds (dihydroxybenzoic acid) appeared to slow platelets from clumping together. So in theory, stacking the two could nudge your bleeding risk a little higher than aspirin alone.

I'll be straight with you though, this effect has never actually been reported in people, so the evidence here is weak. Still, it's worth a simple heads-up if you notice easy bruising, nosebleeds, bleeding gums, or cuts that take a while to stop. Before adding eleuthero, just run it by your pharmacist or doctor so they can weigh in for your situation.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP2D6.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP2D6. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP2D6 drug metabolism.

Here's a small wrinkle worth knowing. The codeine in your combo is actually a prodrug, meaning your body has to convert it into morphine using an enzyme called CYP2D6 before it can ease your pain. Lab and animal studies hinted that eleuthero might slow this enzyme down. If that were true, you'd expect less pain relief from the codeine, since less of it would turn into morphine.

The reassuring part: a real study in healthy people taking eleuthero twice daily for two weeks showed no meaningful effect on CYP2D6. So this concern stays mostly theoretical and is rated minor and unlikely. The aspirin side isn't involved here. Still, if you notice your pain isn't controlled like usual, mention it to your pharmacist or doctor.
