Here's the thinking behind this one. Azathioprine works by quieting your immune system, which is exactly what you want whether you're protecting a transplanted kidney or calming the joint attacks of rheumatoid arthritis. Eleuthero, on the other hand, has been studied as something that may rev up immune activity. So in theory the two could work against each other, and eleuthero might blunt some of the benefit you're getting from azathioprine.

I want to be honest about the evidence, though: this concern comes from lab dishes and animal studies, not from people, so it's more of a caution than a proven problem. Still, since azathioprine is doing such an important job, I'd run any eleuthero product by your transplant team, rheumatologist, or pharmacist before starting it. Please don't change your azathioprine on your own.

About Azathioprine

What Is Azathioprine Used For?

Brand names: Imuran, Azamune

Azathioprine is used alongside other medications to help prevent the body from rejecting a kidney transplant (renal homotransplantation). It is also used to treat active rheumatoid arthritis — reducing signs and symptoms in adults who have not responded well enough to other treatments.

Other pain relievers such as aspirin, NSAIDs, or low-dose steroids may be continued alongside azathioprine for rheumatoid arthritis. Combining azathioprine with disease-modifying antirheumatic drugs (DMARDs) has not been well studied and is not recommended.
