Eleuthero and Belladonna, Caffeine, Ergotamine, Pentobarbital

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Eleuthero interacts with whole categories of medication. Belladonna, Caffeine, Ergotamine, Pentobarbital falls into 2 of those categories, so it can interact with Eleuthero in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Belladonna, Caffeine, Ergotamine, Pentobarbital

Belladonna, Caffeine, Ergotamine, Pentobarbital falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP1A2.
In vitro and animal research suggest that standardized extracts of eleuthero inhibit CYP1A2. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your combination product has several ingredients, but the one that matters here is the caffeine. Your body clears caffeine using a liver enzyme called CYP1A2. In lab dishes and animal studies, eleuthero appears to slow that enzyme down. If the same thing happened in people, caffeine could linger longer and build up, which might mean more jitteriness, a racing or pounding heart, trouble sleeping, or feeling shaky or anxious.

I'll be honest with you though: this is only a theoretical concern. It has been seen in test tubes and animals, never confirmed in humans, so the real-world risk is likely small. Still, if you notice extra restlessness or palpitations, mention it to your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Belladonna, Caffeine, Ergotamine, Pentobarbital also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combo product (Micomp PB) is a mix of belladonna, caffeine, ergotamine, and a barbiturate (pentobarbital), often used for migraine-type headaches. The ergotamine piece is the one handled by the CYP3A4 enzyme in your liver. The thought is that eleuthero might slow that enzyme down, which could let ergotamine build up a bit and theoretically raise the chance of side effects like tingling, cold hands and feet, nausea, or muscle cramps.

The reassuring part: this is mostly lab and animal data. When researchers actually gave eleuthero to healthy people for two weeks, it did not meaningfully block CYP3A4. So a real problem is unlikely. Still, ergotamine is a touchy drug, so run this past your pharmacist or doctor before combining them.

About Eleuthero

What is Eleuthero?

Eleuthero
  • Fatigue and low energy
  • Physical and athletic performance
  • Stress resistance (adaptogen)
  • Immune support
  • Mental focus and alertness

Eleuthero is an herb traditionally used as an 'adaptogen' to fight fatigue, boost energy, and help the body handle stress. The scientific evidence behind these uses is limited and mixed, so it should not replace proven treatments. It is generally well tolerated short-term, but pregnant and breastfeeding people and those with certain conditions should avoid it or check with a healthcare provider first.
