Why this applies to Irinotecan Hydrochloride

Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the gist with eleuthero and your irinotecan (Onivyde). Irinotecan is partly handled by a liver enzyme called CYP3A4. The concern on paper is that eleuthero might slow that enzyme down, which could let irinotecan build up a bit and possibly add to its side effects, things like diarrhea, low blood counts, nausea, and fatigue.

The good news is this worry comes mostly from test tube and animal studies. When researchers actually gave eleuthero to healthy people, it did not meaningfully block CYP3A4. So the real-world risk looks low. Still, because irinotecan is potent chemo, please run any eleuthero use by your oncology team or pharmacist before starting it.

Irinotecan Hydrochloride also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Theoretically, eleuthero might decrease levels of drugs metabolized by OATP.
In vitro research suggests that eleuthero inhibits OATP2B1, which might reduce the bioavailability of oral drugs that are substrates of OATP2B1. Due to the weak inhibitory effect identified in this study, this interaction is not likely to be clinically significant.

Here's the short version: this is a very weak, theory-only concern. In a lab dish, eleuthero slightly blocked a transporter called OATP2B1, which helps move certain drugs into the body. On paper, blocking it could lower how much of an OATP drug gets absorbed. But the effect seen was tiny, and irinotecan (Onivyde) isn't even something you swallow, it's given as an IV infusion in your cancer clinic, so absorption from the gut isn't really the issue here.

Because this is based only on test-tube data and irinotecan is a serious chemotherapy drug, I'd play it safe. Don't add eleuthero on your own while on treatment. Run any supplement by your oncology team or pharmacist first so nothing interferes with your therapy.
