Rifabutin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the short version on combining eleuthero with rifabutin. Rifabutin is handled by a liver enzyme called CYP3A4. In test tube and animal studies, eleuthero looked like it might slow that enzyme down a little, which in theory could let rifabutin build up and raise the odds of side effects like nausea, an upset stomach, a harmless orange or reddish tint to your urine and tears, or changes in your blood counts.

But here's the reassuring part: when researchers gave eleuthero to healthy people for two weeks, it did not actually meaningfully affect CYP3A4. So this is mostly a theoretical concern, rated minor and unlikely. Since rifabutin treats serious infections, it's still smart to mention any supplement to your pharmacist or doctor before adding it.

Brand name: Mycobutin

Rifabutin capsules are used to prevent disseminated MAC (Mycobacterium avium complex) disease — a serious bacterial infection that can spread throughout the body — in adults with advanced HIV infection.

It is not approved as a treatment for active tuberculosis (TB), and it should not be used alone for MAC prophylaxis in anyone who has active TB.
