Riociguat falls under P-GLYCOPROTEIN SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of P-glycoprotein substrates.
In vitro research suggests that eleuthero can inhibit the multi-drug transporter protein, P-glycoprotein. However, it is too soon to tell if this is clinically important. This interaction has not been reported in humans.

Here's the situation with Adempas (riociguat) and eleuthero. Riociguat gets moved around your body partly by a transporter called P-glycoprotein, which acts like a doorway helping carry the drug out. Lab studies hint that eleuthero may slow that doorway down. If that happened in real life, more riociguat could stay in your system, which might nudge up its blood-pressure-lowering effects. That could mean more dizziness, lightheadedness, headache, or low blood pressure.

I want to be honest, though: this is only based on test-tube research, and it has not been seen in actual patients. So it's more of a theoretical caution than a proven problem. Since riociguat already drops blood pressure noticeably, it's worth a quick chat with your pharmacist or doctor before adding eleuthero.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the gist for Riociguat (Adempas): your body relies partly on an enzyme called CYP3A4 to clear this medicine out. Lab and animal studies hinted that eleuthero might slow that enzyme down a bit. If that happened, riociguat could build up in your system, which might mean more low blood pressure, dizziness, lightheadedness, headache, or upset stomach.

The reassuring part is that when researchers actually tested eleuthero in healthy people, it did not meaningfully block CYP3A4. So this is a theoretical, unlikely concern, not a proven problem. Still, since riociguat already affects blood pressure, it's worth a quick chat with your pharmacist or doctor before adding eleuthero, just so they can keep an eye on how you feel.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold under the brand names Adempas and Riociguat — are approved for adults with two types of high blood pressure in the lungs. The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots that have blocked or narrowed the arteries in the lungs. Riociguat is used when surgery has not fully fixed the problem, or when surgery is not an option.

The second is pulmonary arterial hypertension (PAH), a progressive disease where the arteries in the lungs become stiff and narrowed. In PAH, riociguat helps improve exercise ability, functional class (how well you can get around day-to-day), and slows disease worsening. It can be used alone or combined with other PAH medicines called endothelin receptor antagonists or prostanoids.
