Why this applies to Telaprevir

Telaprevir falls under P-GLYCOPROTEIN SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of P-glycoprotein substrates.
In vitro research suggests that eleuthero can inhibit the multi-drug transporter protein, P-glycoprotein. However, it is too soon to tell if this is clinically important. This interaction has not been reported in humans.

Here's the short version: your body uses a kind of cellular bouncer called P-glycoprotein to push certain drugs back out of cells and into the gut. Telaprevir is one of the drugs that bouncer handles. In test-tube studies, eleuthero seemed to slow that bouncer down, which in theory could let a bit more telaprevir stay in your system. If that happened, you might notice more of telaprevir's usual side effects, like nausea, itching, rash, or hemorrhoid-type discomfort.

The key word here is theoretical. This was only seen in lab studies, not in real people, so we honestly don't know if it matters in everyday life. Still, since telaprevir is a powerful hepatitis C medicine, please run eleuthero past your pharmacist or doctor before combining the two.

Telaprevir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the gist for telaprevir (Incivek), the hepatitis C medicine. It's cleared from your body partly by a liver enzyme called CYP3A4. The thought is that eleuthero might slow that enzyme down, which in theory could let telaprevir build up a little and bring on more of its usual side effects, like rash, itching, nausea, or low red blood cell counts (anemia).

The good news: this is a theoretical worry. It showed up in lab and animal tests, but when actual people took eleuthero for two weeks, it did not meaningfully affect CYP3A4. So a real-world problem here is unlikely. Still, since telaprevir is part of a serious, carefully managed treatment, just mention any supplement you're taking to your pharmacist or doctor.

About Telaprevir

What Is Telaprevir Used For?

  • Telaprevir is used along with two other medications (ribavirin [Copegus, Rebetol] and peginterferon alfa [Pegasys]) to treat chronic hepatitis C (an ongoing viral infection that damages the liver) in people who have not yet been treated for this condition or whose condition could not successfully be treated with ribavirin and peginterferon alfa alone. Telaprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Telaprevir may not prevent the spread of hepatitis C to other people.
