Dr. Brian Staiger explains PharmD, Clinical Pharmacist

First things first: I'd steer you away from European mandrake entirely. It's a genuinely toxic nightshade plant, and there's no safe, proven dose. So my strongest advice is simply not to take it, with or without your medication.

As for the interaction itself, it's only a theoretical one based on how these plants work. Mandrake's tropane alkaloids (like atropine) slow down your gut, and when your gut moves more slowly, an oral drug can sit there and get absorbed more fully. In theory, that could mean a bit more hydroxyprogesterone ending up in your system. Hydroxyprogesterone works as-is (it isn't a prodrug needing activation), so more absorption could nudge up effects like drowsiness, headache, breast tenderness, or mood changes. Please run this by your doctor or pharmacist before mixing anything.

From our pharmacist’s overview on the Hydroxyprogesterone page

“Hydroxyprogesterone (Hylutin, Makena) is a progesterone hormone often prescribed to help sustain a pregnancy, which raises the stakes on this list considerably. Nearly everything here does the same thing: fibers, gums, and mucilage-rich herbs, psyllium being the everyday example, can cut how much of an oral dose your body actually abso…”
