First, a clear safety note: European mandrake is genuinely toxic, and I would not recommend taking it in any form. Setting that aside, here is the theoretical concern with your Orenitram (treprostinil). Mandrake's tropane alkaloids slow down how fast your gut moves things along. When the gut slows, an oral medication can sit longer and potentially be absorbed more fully, which could raise the amount of treprostinil in your system.

With treprostinil, more drug usually means more of its typical side effects, things like headache, flushing, nausea, diarrhea, jaw pain, and lightheadedness. This is only a theoretical interaction based on how these chemicals behave, not proven in studies. Treprostinil is not a prodrug. Please don't change anything on your own, just check with your pharmacist or doctor.

European mandrake is a highly toxic plant from the nightshade family that was used in ancient and medieval times as a sedative and pain reliever. It contains powerful poisons that can cause serious harm or death, and there is no proof it safely treats any condition. We strongly recommend you do not consume mandrake in any form.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
