Why this applies to Apalutamide

Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Evodia interacts with.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the idea behind this one. Apalutamide (Erleada) is processed in your liver partly by an enzyme called CYP3A4. Lab studies suggest Evodia may slow that enzyme down. In theory, that could let more apalutamide build up in your body, which might mean stronger effects and a higher chance of its usual side effects, things like fatigue, joint pain, rash, high blood pressure, or diarrhea.

I want to be honest about how soft this evidence is. So far this has only been seen in test tube research, not in real people, so we genuinely don't know if it matters in everyday life. Still, since apalutamide is a serious cancer medicine, please don't start Evodia without first checking with your oncology team or pharmacist.

Apalutamide also falls under CYTOCHROME P450 3A4 (CYP3A4) INDUCERS.

Theoretically, taking CYP3A4 inducers might decrease the levels and clinical effects of evodia.
Animal research shows that concomitant administration of dexamethasone, a known CYP3A4 inducer, with the alkaloid constituents of evodia significantly reduces the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.

Here's the thing to keep straight with this pairing: it's not your Apalutamide (Erleada) that's likely to change, it's the Evodia. Apalutamide is a strong CYP3A4 inducer, meaning it speeds up the liver enzyme that breaks down many substances. Evodia's active compounds (like evodiamine and rutaecarpine) appear to be cleared by that same enzyme.

So if you take them together, your body may chew through the Evodia faster, leaving lower levels and possibly less of whatever benefit you were hoping for. The honest catch is that this is based only on lab and animal studies, not people, so it's more of a theoretical concern than a proven one. Either way, please run any supplement past your oncology team before adding it to your treatment.

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
