Astemizole falls under QT INTERVAL-PROLONGING DRUGS — the group Evodia interacts with.

Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the concern with this pairing. Astemizole is an older antihistamine that's well known for affecting the heart's electrical timing, something doctors call the QT interval. When the QT gets stretched too far, it can rarely trigger a dangerous heart rhythm called Torsade de pointes. In fact, this very risk is why astemizole was pulled from the U.S. market years ago.

Lab and animal studies suggest Evodia may lengthen the QT interval too. So stacking the two could, in theory, add up and raise the chance of a serious rhythm problem like a racing or irregular heartbeat, dizziness, or fainting. To be clear, this is theoretical and based on animal data, not proven in people. Still, because astemizole is so heart-sensitive, please run this by your pharmacist or doctor before combining them.
