Dextromethorphan, Guaifenesin, Phenylpropanolamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Evodia interacts with.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the simple version. In this combo cough and cold product, the dextromethorphan piece is the one that leans on a liver enzyme called CYP3A4 to get cleared from your body. In lab dishes, evodia appears to slow that enzyme down. If that same thing happened in people, your body might clear dextromethorphan a little more slowly, so it could build up and bring on more of its usual effects like drowsiness, dizziness, or feeling foggy.

The big caveat: this has only been seen in test tubes, never confirmed in actual people, so it's mostly theoretical. The guaifenesin and decongestant parts aren't really involved here. Still, before pairing evodia with this medicine, it's worth a quick chat with your pharmacist or doctor.
