Animal research shows that concomitant administration of ketoconazole, a known CYP3A4 inhibitor, with the alkaloid constituents of evodia significantly increases the area under the curve (AUC), maximum concentration (Cmax), and half-life of these constituents.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's some good news if you're using Vusion (a diaper rash cream that mixes miconazole, zinc oxide, and petrolatum). This product is applied to the skin, not swallowed. Very little of it gets into your bloodstream, so it isn't going to meaningfully block the liver enzyme (CYP3A4) that this evodia interaction depends on.
The concern on file is purely theoretical and comes from animal studies using ketoconazole, a strong oral antifungal, not a topical cream. A skin cream simply doesn't reach the levels needed to slow how your body clears evodia. Realistically, your main thing to watch with Vusion is local skin reactions like redness or irritation. Still, since evodia itself has thin human evidence, it's smart to run any supplement past your pharmacist or doctor.

In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's some reassuring news about this combination. The product you're asking about (Vusion) is a skin cream that combines miconazole, zinc oxide, and petrolatum, and it's used right on the skin to treat diaper rash with a yeast component. Because you apply it topically, very little gets into your bloodstream.
The interaction on file is about evodia possibly slowing down a liver enzyme (CYP3A4) that breaks down certain drugs, which could let those drugs build up. But that only matters for medicines your liver has to process. Since this cream stays mostly on the skin's surface, that liver interaction realistically won't apply here. On top of that, the evidence is only from lab studies, not people. I'd still mention any supplements to your pharmacist, but I wouldn't lose sleep over this pairing.
