Naproxen Sodium falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Evodia interacts with.

Evodia might reduce the levels and clinical effects of CYP1A2 substrates through induction of CYP1A2.
Evodia extract and the evodia constituent rutaecarpine induce hepatic CYP1A2 enzyme activity. Evodia decreases levels of theophylline and caffeine, CYP1A2 substrates, by about 70% in animal models.

Here's the gist: naproxen (the active ingredient in Midol Extended Relief) is partly handled by a liver enzyme called CYP1A2. Evodia appears to speed that enzyme up, which could clear naproxen out of your system a little faster than usual. Naproxen is the real, working drug here (not a prodrug), so faster clearance might mean it works less well, leaving you with more lingering pain, cramps, or a headache that isn't fully controlled.

How worried should you be? Not very. This is based only on lab and animal studies, so it's a theoretical concern, not something proven in people. Still, if you're using both regularly and your pain relief seems weaker, mention it to your pharmacist or doctor before changing anything.

Theoretically, taking evodia with antiplatelet or anticoagulant drugs might increase the risk of bruising and bleeding.
In vitro and animal studies show that rutaecarpine, a constituent of evodia, inhibits platelet aggregation.

Here's the gist: naproxen (the pain reliever in Midol Extended Relief) already nudges your blood toward clotting a little slower and can irritate the stomach. Evodia may do something similar. One of its compounds, rutaecarpine, has slowed down platelet clumping in lab and animal studies, which is the same general direction naproxen pushes. Stack the two and you could, in theory, see a bit more bruising, nosebleeds, or stomach upset and bleeding.

That said, this is weak evidence. It comes from test tubes and animals, not people, so the real-world risk is uncertain. If you take naproxen regularly or have any bleeding history, just run the combo past your pharmacist or doctor before adding Evodia.
