Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.

Here's the thing to know about pairing Evodia with ranolazine (Ranexa): ranolazine can lengthen what doctors call the QT interval, which is part of your heart's electrical rhythm. In lab and animal studies, evodia has done the same thing. Putting two QT-stretching agents together could, in theory, add up and raise the chance of an irregular or dangerous heartbeat.

I want to be honest about the evidence, though. This concern comes from test tube and animal research, not from people taking both together, so it's more of a caution than a proven problem. Still, since heart rhythm is involved, I'd play it safe. Please check with your pharmacist or doctor before combining them, rather than stopping or changing anything on your own.

Ranolazine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, evodia might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.

Here's the simple version: your body relies on a liver enzyme called CYP3A4 to clear ranolazine out of your system. In lab studies (test tubes, not people yet), evodia appears to slow that same enzyme down. If that held true in the body, ranolazine could pile up a bit higher than usual.

Why does that matter? Ranolazine can affect your heart's rhythm (it can stretch the QT interval), and higher levels may bring on more dizziness, lightheadedness, nausea, or constipation. The good news is this concern is theoretical right now and hasn't been shown in humans. Still, since ranolazine and heart rhythm are involved, I'd play it safe. Please check with your pharmacist or doctor before pairing evodia with it, rather than starting or stopping anything on your own.

What Is Ranolazine Used For?

Brand names: Ranexa, Aspruzyo

Ranolazine treats chronic angina — the recurring chest pain or pressure that happens when the heart isn't getting enough blood and oxygen. It is used when other angina medicines aren't enough on their own.

Both the extended-release tablet (sold as Ranexa and generic ranolazine) and the oral powder form (sold as Aspruzyo Sprinkle) are approved for this use. Ranolazine can be combined with other heart medicines such as beta-blockers, nitrates, calcium channel blockers, anti-platelet drugs, statins, ACE inhibitors, and angiotensin receptor blockers.
