Evodia + Voclosporin
Voclosporin brand name: Lupkynis.
Voclosporin falls into 2 drug categories that Evodia can interact with; the most serious is rated moderate. Theoretically, evodia might have an additive effect with drugs that prolong the QT interval, potentially increasing the risk of ventricular arrhythmias.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Evodia interacts with whole categories of medication. Voclosporin falls into 2 of those categories, so it can interact with Evodia in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
QT INTERVAL-PROLONGING DRUGS
1 of 2Why this applies to Voclosporin
Voclosporin falls under QT INTERVAL-PROLONGING DRUGS — the group Evodia interacts with.
Evodia has demonstrated dose-dependent activity as a proarrhythmic agent in animal and in vitro studies. Evodia infusion in animals extends the action duration potential and induces prolongation of the QT interval and Torsade de pointes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing Evodia and Voclosporin (Lupkynis). Voclosporin can affect your heart's electrical timing, something we call QT prolongation. Evodia, in lab and animal studies, has shown it can do the same thing. Stack two QT-prolonging influences together and you may, in theory, raise the risk of an irregular, potentially dangerous heart rhythm.
I want to be honest about the strength here: this is based on test tube and animal research, not human studies, so it's more of a theoretical caution than a proven problem. Still, since Voclosporin already carries a QT warning, I'd play it safe. Please don't change your Voclosporin, but do check with your pharmacist or doctor before adding Evodia.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Voclosporin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that evodia extract inhibits hepatic CYP3A4. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for you. Your body clears voclosporin mainly through a liver enzyme called CYP3A4. Lab studies suggest evodia may slow that enzyme down. If that happened in people, voclosporin could build up to higher levels than intended, which can mean more side effects like high blood pressure, kidney strain, headache, swelling, or higher potassium.
I want to be honest about the evidence though. This has only been seen in test tubes, not in actual patients, so it's theoretical for now. Still, voclosporin is a powerful immunosuppressant where dosing is carefully controlled, so I'd play it safe. Please check with your pharmacist or doctor before adding evodia, rather than starting or stopping anything on your own.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
- AHigh-quality meta-analysis (quantitative systematic review).
- BCase-control study.
- CConsensus.

About Voclosporin
What Is Voclosporin Used For?
Brand name: Lupkynis
Voclosporin, sold as the capsule Lupkynis, treats active lupus nephritis in adults. This is kidney inflammation caused by lupus.
It is always used together with a background immunosuppressive regimen, not on its own. Its safety and effectiveness have not been established with cyclophosphamide, so that combination is not recommended.
