Acetaminophen, Caffeine, Codeine, Salicylamide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with this combo: the codeine part of your Codalan is a prodrug. Your body has to switch it on using an enzyme called CYP2D6, turning it into morphine, which is what actually relieves your pain. In lab (test tube) studies, False Unicorn appears to slow that same enzyme down.

So in theory, False Unicorn could mean less codeine gets converted to its active form, leaving you with weaker pain relief than expected. The other ingredients (acetaminophen, caffeine, salicylamide) aren't really affected here. Keep in mind this is only based on lab data, not real people, so it may not amount to much. Still, if you take this medicine regularly, it's worth a quick chat with your pharmacist or doctor before adding False Unicorn.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

This combination pill has several pieces, but the one I'd watch is the codeine. Your body has to turn codeine into morphine to relieve pain, and a big part of that activation runs through liver enzymes. False Unicorn has only been shown in test-tube studies to slow one of those enzymes (CYP3A4), so any real-world effect is theoretical and far from proven.

If it did matter, slowing this pathway could shift how much active morphine you make from codeine, which might mean weaker pain relief or, in some people, a bit more sleepiness or constipation. Honestly, with such weak evidence, I wouldn't lose sleep over it. Still, since safety data on False Unicorn is thin, please run it past your pharmacist or doctor first.
