Amitriptyline, Perphenazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Your medication combines amitriptyline (an antidepressant) with perphenazine (an antipsychotic). Both are cleared, at least in part, by a liver enzyme called CYP2D6. In a lab dish, False Unicorn appeared to slow that enzyme down. If the same thing happened in your body, these two drugs could clear more slowly and build up a bit, which might mean stronger side effects like heavy drowsiness, dry mouth, constipation, blurry vision, dizziness on standing, or a faster heartbeat.

Here's the honest part: this is based only on a test-tube study, not on real people, so it may not matter much in practice. Still, since this is a sensitive combination, I'd keep your pharmacist or doctor in the loop before adding False Unicorn rather than starting it on your own.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

This combination pill pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an antipsychotic), and your body partly relies on an enzyme called CYP3A4 to break both of them down. In a lab dish, False Unicorn appeared to slow that enzyme. If the same thing happened in a person, these medicines could clear a bit more slowly and build up, which might mean stronger drowsiness, dry mouth, constipation, dizziness on standing, blurred vision, or a faster heartbeat.
