Clomipramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the gist for Clomipramine (Anafranil). Your body relies on a liver enzyme called CYP2D6 to help break this medication down. In lab testing (not in people), False Unicorn appeared to slow that enzyme. If that held true in real life, your Clomipramine could clear out more slowly and build up a bit, which may mean stronger effects and more of its usual side effects, things like a dry mouth, constipation, drowsiness, dizziness, a faster heartbeat, or feeling more sedated.

I want to be honest though: this is based only on a test-tube study, so it's a theoretical concern, not a proven one. Still, because tricyclics like this one can be touchy with dosing, it's worth a quick chat with your pharmacist or doctor before combining them.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for Clomipramine (Anafranil): your body relies partly on a liver enzyme called CYP3A4 to break this medication down. In a lab dish, False Unicorn appears to slow that enzyme down. If that happened in real life, clomipramine could clear out more slowly and build up a bit higher than usual. Since clomipramine is an active drug (not a prodrug), more of it staying around means a stronger effect, which could mean more side effects like drowsiness, dry mouth, constipation, dizziness, or a faster heartbeat.

That said, this is only a theoretical concern based on test-tube data, not real patients, so the actual risk is uncertain. Still, because clomipramine affects your heart and nervous system, I'd run it by your pharmacist or doctor before combining the two.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.
