Hydrocodone Bitartrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group False Unicorn interacts with.

In vitro, false unicorn extract inhibits cytochrome P450 2D6 (CYP2D6) activity. Theoretically, false unicorn might increase the levels of CYP2D6 substrates. Some of drugs that are CYP2D6 substrates include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and others.

Here's the wrinkle with hydrocodone. Your liver uses an enzyme called CYP2D6 to turn hydrocodone into hydromorphone, which is actually a stronger painkiller. In a test-tube study, False Unicorn appeared to slow down that same enzyme. If that happened in your body, you might convert less hydrocodone into its more potent form, so you could notice weaker pain relief than usual.

That said, this is only lab data, not real patient evidence, so we honestly don't know how much it matters in people. I wouldn't panic, but I also wouldn't quietly add this herb on your own. Please run it past your pharmacist or doctor first, especially since your pain is being managed around the clock.

In vitro, false unicorn extract inhibits cytochrome P450 3A4 (CYP3A4) activity. Theoretically, false unicorn might increase the levels of CYP3A4 substrates. Drugs that might be affected include some calcium channel blockers (diltiazem, nicardipine, verapamil), chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), glucocorticoids, cisapride (Propulsid), alfentanil (Alfenta), fentanyl (Sublimaze), losartan (Cozaar), fluoxetine (Prozac), midazolam (Versed), omeprazole (Prilosec), ondansetron (Zofran), propranolol (Inderal), fexofenadine (Allegra), and numerous others.

Here's the gist for your hydrocodone. In lab dish studies, False Unicorn appears to slow down an enzyme called CYP3A4, which is one of the pathways your liver uses to clear hydrocodone. If that enzyme gets blocked, hydrocodone could linger longer in your body, and its effects could stack up more than expected. With a strong opioid, that could mean extra drowsiness, dizziness, constipation, slowed breathing, or feeling overly sedated.

That said, this is only a theoretical concern based on test-tube data, not real people, so we honestly don't know if it matters in everyday life. Still, hydrocodone is potent and not something to take chances with. Please check with your pharmacist or doctor before pairing the two so they can weigh in on your situation.
