Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Orencia (abatacept): this medicine works by calming down an overactive immune system to ease rheumatoid arthritis pain, swelling, and joint damage. Fermented milk (think yogurt or kefir) contains live probiotics that some lab and clinical research suggests can gently stimulate the immune system. In theory, that little immune nudge could work against what abatacept is trying to do, possibly making it slightly less effective.

I want to be honest, though: this is a theoretical concern based on how things work in the body, not strong proof of a real problem. Everyday food amounts of yogurt are very unlikely to undo your treatment. If you take concentrated probiotic supplements or have a weakened immune system, just mention it to your doctor or pharmacist so they can keep an eye on things.

“Abatacept (Orencia) treats rheumatoid arthritis and related conditions by dialing down immune activity, so the supplements that matter here are the ones pulling in the opposite direction. All 53 ingredients on this list share that single theme: herbs and mushrooms with immune-stimulating properties, echinacea, ashwagandha, and astragal…”
