Here's the situation with Zilucoplan (Zilbrysq), the injectable medicine used to treat generalized myasthenia gravis. It works by quieting one specific part of your immune system (the complement pathway) to help reduce muscle weakness. Fermented milk products like yogurt or kefir may have a mild immune-stimulating effect. The theory is that revving up the immune system could work against a medicine designed to calm it down, possibly making Zilucoplan a bit less effective.

I want to be honest though: this is a theoretical concern based on how these things behave in the lab, not something proven in people taking Zilucoplan. Normal food amounts of yogurt are very unlikely to cause a problem. Zilucoplan is not a prodrug, so this is not about how it's activated. If you enjoy fermented milk regularly, just mention it to your neurologist or pharmacist so they can keep an eye on your symptoms.
