Caffeine , Sodium Benzoate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group German Chamomile interacts with.
In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: lab studies hint that German chamomile might slow down a liver enzyme called CYP3A4, which helps clear certain medications. If that enzyme gets blocked, a drug it processes could build up a bit and hit harder. With caffeine and sodium benzoate, that could mean more of caffeine's usual effects, think jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired.
That said, I don't want to overstate this. This is only test-tube evidence, and it has never actually been reported in real patients. Caffeine is also handled mostly by a different enzyme (CYP1A2), so a real-world effect here is uncertain. A cup of chamomile tea is unlikely to cause trouble, but if you're using concentrated supplements, just run it by your pharmacist or doctor first.

Here's the simple version. The caffeine in this medicine is cleared from your body mainly by a liver enzyme called CYP1A2. German chamomile might slow that enzyme down a little, at least based on lab and animal studies. If that happened in a real person, caffeine could hang around longer and feel a bit stronger, meaning more jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired.
That said, I want to keep this in perspective. This is a theoretical, low-level concern that has not actually been reported in people, and the effect is rated minor. Casual chamomile tea is very unlikely to cause trouble. If you use concentrated chamomile supplements and notice extra caffeine side effects, just check in with your pharmacist or doctor.
