Irinotecan Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group German Chamomile interacts with.

Theoretically, German chamomile might inhibit CYP3A4 and increase levels of drugs metabolized by these enzymes.
In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.

Here's the short version with your irinotecan (Onivyde). Your body uses an enzyme called CYP3A4 to help break this chemo drug down. German chamomile, in lab dish studies, has shown it might slow that enzyme. If that happened in real life, irinotecan could build up a bit and hit harder than expected, which for this drug can mean worse diarrhea, low blood counts, nausea, and fatigue.

Now the important part: this is only test-tube evidence, and it has not been seen in actual patients. Casual cups of chamomile tea are unlikely to be a big deal, but concentrated supplements are less predictable. Since chemo dosing is so precise, please run any chamomile use past your oncology team or pharmacist before adding it.
