Riociguat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group German Chamomile interacts with.

Theoretically, German chamomile might inhibit CYP3A4 and increase levels of drugs metabolized by these enzymes.
In vitro evidence shows that German chamomile might inhibit CYP3A4. So far, this interaction has not been reported in humans. However, there might be an increase in the levels of drugs metabolized by CYP3A4 in patients taking German chamomile.

Here's the gist for Adempas (riociguat). Your liver uses an enzyme called CYP3A4 to help clear this medicine out of your body. German chamomile, in lab dish studies, looks like it might slow that same enzyme down. If that happened in a real person, riociguat could build up a bit higher than expected.

Since riociguat already works by relaxing blood vessels and lowering blood pressure, extra levels could mean more low blood pressure, dizziness, lightheadedness, or headache. The honest part: this is only a theoretical concern from test-tube data, not something seen in actual patients, and an occasional cup of chamomile tea is unlikely to cause trouble. Still, before using regular chamomile supplements, run it by your pharmacist or doctor.

It is generally well tolerated, but people allergic to ragweed or related plants and those who are pregnant should be cautious.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
