Levomilnacipran falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for Levomilnacipran (Fetzima): your body partly clears this antidepressant using a liver enzyme called CYP3A4. The berberine in goldthread may slow that enzyme down, at least in lab studies. If it does, levomilnacipran could clear a bit more slowly and build up in your system. That might mean stronger effects and more side effects, like a faster heart rate, higher blood pressure, nausea, sweating, trouble sleeping, or feeling jittery.

I want to be honest about the evidence here: this is theoretical. It's based on how the chemicals behave in the lab, not on actual reports in people. So it's worth a heads-up, not panic. Before adding goldthread, just check with your pharmacist or doctor so they can weigh it for you.
