Telaprevir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for telaprevir (Incivek). Your body clears this hepatitis C medicine largely through a liver enzyme called CYP3A4. Goldthread contains berberine, which lab work suggests might slow that same enzyme down. If it does, telaprevir could clear more slowly and build up a bit, which can mean more of its usual side effects like itchy skin rashes, nausea, diarrhea, and a low red blood cell count.

I want to be honest about the evidence: this is theoretical, based on how berberine behaves in the lab, and it has not actually been reported in people. Still, since telaprevir is part of a careful hepatitis C regimen, I'd run goldthread past your pharmacist or doctor before adding it rather than guessing.

About Telaprevir

What Is Telaprevir Used For?

  • Telaprevir is used along with two other medications (ribavirin [Copegus, Rebetol] and peginterferon alfa [Pegasys]) to treat chronic hepatitis C (an ongoing viral infection that damages the liver) in people who have not yet been treated for this condition or whose condition could not successfully be treated with ribavirin and peginterferon alfa alone. Telaprevir is in a class of medications called protease inhibitors. It works by decreasing the amount of hepatitis C virus (HCV) in the body. Telaprevir may not prevent the spread of hepatitis C to other people.
