Ondansetron falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for ondansetron (Zofran). Your liver uses an enzyme called CYP3A4 to help clear this drug from your body. Berberine, the active piece of goldthread, may slow that enzyme down a bit. If that happens, ondansetron could hang around a little longer than usual, which in theory could nudge up its typical side effects like headache, constipation, or feeling lightheaded.

I want to be honest about the evidence here: this is theoretical, based on how berberine behaves in the lab, and it hasn't actually been reported in people. Ondansetron is usually taken for short stretches, which lowers the concern even more. Still, since goldthread supplements are far more concentrated than any food, it's worth a quick chat with your pharmacist or doctor before combining them.

What Is Ondansetron Used For?

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
