Ondansetron falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin E interacts with.

Theoretically, vitamin E might induce metabolism of CYP3A4, possibly reducing the levels CYP3A4 substrates.
Vitamin E appears to bind with the nuclear receptor, pregnane X receptor (PXR), which results in increased expression of CYP3A4. Although the clinical significance of this is not known, use caution when considering concomitant use of vitamin E and other drugs affected by these enzymes.

Here's the gist with ondansetron (Zofran) and vitamin E. Your liver uses an enzyme called CYP3A4 to help clear ondansetron from your body. The thought is that high-dose vitamin E might gently speed up that enzyme, which could clear ondansetron a little faster and, in theory, make it slightly less effective at keeping nausea and vomiting away.

I want to be straight with you, though: this is purely theoretical, based on how the chemicals behave in the lab, not on real patient problems. Ondansetron is not a prodrug, so it doesn't flip the other way. Everyday food amounts of vitamin E won't matter here. If you're taking high-dose supplements, just mention it to your pharmacist or doctor so they can keep an eye on whether your nausea control is holding up.

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
