Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Horny Goat Weed interacts with.

Theoretically, horny goat weed might increase the effects and side effects of CYP1A2 substrates.
In vitro, horny goat weed leaf extract inhibits CYP1A2. This effect has not been reported in humans.

Here's the gist for ondansetron (Zofran). Your liver uses a few enzymes to clear this medicine, and one of them is called CYP1A2. In test tube studies, horny goat weed appears to slow down that enzyme. If that happened in the body, ondansetron could hang around a little longer, which might mean slightly stronger effects and more chance of side effects like headache, constipation, dizziness, or tiredness.

The important part: this is only lab data, and it has never been shown to happen in real people. So I wouldn't lose sleep over it. Still, since horny goat weed itself isn't well studied for safety, it's worth a quick chat with your pharmacist or doctor before combining the two.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist for ondansetron (Zofran). Your body uses an enzyme called CYP3A4 to help clear this drug. In lab dish studies, horny goat weed seemed to slow that same enzyme down. If that held true in people, ondansetron could hang around a bit longer and you might notice more of its usual effects, like headache, constipation, dizziness, or tiredness.

But here's the important part: this was only seen in test tubes, never in actual people. That's pretty weak evidence, so I wouldn't lose sleep over it. Ondansetron also carries its own small heart rhythm (QT) concern, so before combining the two, it's worth a quick chat with your pharmacist or doctor.

About Ondansetron

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
