Why this applies to Ondansetron

Ondansetron falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Sulforaphane interacts with.

Theoretically, sulforaphane might alter the levels and clinical effects of CYP1A2 substrates.
Animal and in vitro research shows that sulforaphane inhibits CYP1A2 enzyme activity. However, its precursor glucoraphanin also appears to increase the expression of the CYP1A2 enzyme, which could lead to increased metabolism of CYP1A2 substrates. These effects have not been reported in humans.

Here's the gist for ondansetron (Zofran): your liver uses an enzyme called CYP1A2 (along with a few others) to help clear this drug out of your body. Sulforaphane, the compound concentrated from broccoli, has shown two opposite effects on that enzyme in lab and animal tests. It may slow the enzyme down (which could let ondansetron build up a bit, possibly adding to side effects like headache, constipation, or feeling lightheaded), or its precursor may speed the enzyme up (which could weaken how well it controls your nausea).

Honestly though, this is all theoretical right now. It has not been seen in people, and normal broccoli on your dinner plate is very unlikely to matter. If you take a concentrated sulforaphane supplement, just mention it to your pharmacist or doctor so they can keep an eye out.

Here's the gist for these two together. Your body uses an enzyme called CYP3A4 to help clear ondansetron (Zofran). In lab and animal studies, sulforaphane appears to slow that enzyme down. If that happened in people, ondansetron could hang around a bit longer and its effects could be slightly stronger. With ondansetron that might mean more headache, constipation, or dizziness, and in theory a touch more effect on heart rhythm (QT prolongation), though that's rarely an issue at normal doses.

The important thing: this is theoretical and has not been seen in humans. Broccoli and other veggies at dinner are not a concern. A concentrated sulforaphane supplement is the part worth running past your pharmacist or doctor first.

About Ondansetron

What Is Ondansetron Used For?

Brand name: Zofran

Ondansetron is used to prevent nausea and vomiting in several situations. Oral forms (tablets, orally disintegrating tablets, and oral solution) are approved to prevent nausea and vomiting from highly or moderately emetogenic (nausea-causing) cancer chemotherapy, from radiation therapy to the abdomen or whole body, and from surgery.

Ondansetron injection (given intravenously or intramuscularly in a clinical setting) is approved to prevent nausea and vomiting from emetogenic cancer chemotherapy — including high-dose cisplatin — in adults and children as young as 6 months old, as well as to prevent postoperative nausea and vomiting.
