Alfentanil falls under CNS DEPRESSANTS — the group Gotu Kola interacts with.

Alfentanil is a strong opioid painkiller, usually given during surgery or procedures, and one of its main effects is slowing down the brain (sedation and slowed breathing). The concern here is that gotu kola may have its own mild calming effect by acting on GABA receptors in the brain. If you stack those two together, you could in theory feel more drowsy, more foggy, or more sedated than you would from the opioid alone.

I want to be honest about the strength of this: it's theoretical, based mainly on lab studies, not real patient cases. Still, with a potent opioid like alfentanil, I'd rather you be cautious. Please mention any gotu kola use to your anesthesia team, pharmacist, or doctor before your procedure rather than stopping anything on your own.
