There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.

Here's the simple version: Fingolimod can already be tough on the liver, which is why your doctor checks your liver enzymes before you start and during treatment. Gotu kola has been linked to a handful of liver problems in case reports too. So the worry isn't that one changes how the other is broken down. It's that two things that can stress the liver might add up if you take them together.

I'll be honest, this concern is theoretical and based on just a few anecdotal reports, so it's not a strong, proven link. Still, since liver safety really matters with fingolimod, I'd skip the gotu kola supplement unless you've checked with your neurologist or pharmacist first. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and report those right away.

What Is Fingolimod Used For?

Brand names: Gilenya, Tascenso ODT

Fingolimod — available as capsules (Fingolimod, Fingolimod Hydrochloride, and Gilenya) and as an orally disintegrating tablet (Tascenso ODT) — is approved for the treatment of relapsing forms of multiple sclerosis (MS) in patients 10 years of age and older. This includes clinically isolated syndrome (a first MS episode), relapsing-remitting MS (the most common type with flare-ups and periods of stability), and active secondary progressive MS (a later stage where disability gradually worsens but relapses still occur).

All supported formulations share these same approved uses. The goal of treatment is to reduce the frequency of relapses and slow disease activity.
