Naltrexone Hydrochloride falls under HEPATOTOXIC DRUGS — the group Gotu Kola interacts with.
There are at least four case reports of hepatotoxicity associated with the use of gotu kola. However, more information is needed to determine if gotu kola was the causative factor in these cases.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing gotu kola and naltrexone. This isn't about one changing the blood level of the other. Instead, both can occasionally be tough on the liver. Naltrexone already carries a liver warning, and your doctor may check liver enzymes while you take it. Gotu kola has been linked to a handful of liver injury reports, though we honestly can't say for sure it was the cause. The worry is that stacking two liver stressors could add up.
To be clear, this is theoretical and based on weak, anecdotal evidence. Still, watch for signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and loop in your pharmacist or doctor before combining them.

About Naltrexone
What Is Naltrexone Used For?
Brand name: Revia
Naltrexone Hydrochloride tablets are used to treat alcohol dependence and to block the effects of opioids in people being treated for opioid dependence. It works best as part of a full treatment program that includes counseling and support.
The extended-release injectable suspension (brand name Vivitrol) is used to treat alcohol dependence in patients who are already not drinking before starting treatment, and to prevent relapse to opioid dependence after a patient has completed opioid detoxification. Vivitrol must always be part of a comprehensive treatment program that includes psychosocial support.
