Here's the gist for remifentanil (Ultiva), which is a powerful opioid given through an IV during surgery or procedures. It already slows your breathing and makes you very drowsy. Lab studies hint that gotu kola may have a calming effect by nudging the same GABA system in the brain that other sedatives act on. So the worry is simple addition: gotu kola could pile onto remifentanil's sedation and make you sleepier, slow your breathing more, or drop your blood pressure.

I'll be honest, this is a theoretical concern based on lab work, not real patient cases. Still, with a drug this strong, it's worth flagging. Since remifentanil is only used in monitored medical settings, please tell your anesthesia team or pharmacist about any gotu kola you take so they can plan around it.
