Linagliptin, Metformin Hydrochloride falls under ANTIDIABETES DRUGS — the group Guava interacts with.

Theoretically, concomitant use with antidiabetes drugs might have additive effects and increase the risk of hypoglycemia. Animal research shows that guava leaf extract or guava fruit can have hypoglycemic effects. Monitor blood glucose levels closely. Medication dose adjustments may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (Diabeta, Glynase PresTab, Micronase), insulin, metformin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the simple version: your Jentadueto XR already does the heavy lifting of lowering your blood sugar, with the metformin and linagliptin working together. Guava, especially concentrated guava leaf supplements, may have its own mild blood-sugar-lowering effect. Stack the two and the worry is that your numbers could drop a little too low. That's called hypoglycemia, and it can show up as shakiness, sweating, dizziness, a fast heartbeat, hunger, or feeling confused.

I'll be honest with you though, this concern comes mostly from animal and lab studies, not solid human research, so it's more of a theoretical caution than a proven danger. Eating guava as a regular fruit is unlikely to cause trouble. If you want to try guava leaf products, check with your pharmacist or doctor first and keep an eye on your home blood sugar readings.
