Dextromethorphan, Guaifenesin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's the gist with this combo. The dextromethorphan part of your cough syrup gets cleared by a liver enzyme called CYP2D6. In lab dishes, higenamine seems to slow that enzyme down. If that also happened in people, dextromethorphan could build up a bit and hit harder, which might mean more dizziness, drowsiness, or feeling foggy and restless. The guaifenesin half isn't really involved here.

The honest part: this is only seen in test-tube studies, not in actual patients, so the real-world risk looks low. Still, higenamine is a stimulant with heart and blood pressure concerns of its own, so I'd run this past your pharmacist or doctor before pairing them.

Here's the gist with this combo. Your cough syrup pairs dextromethorphan (the cough suppressant) with guaifenesin (which loosens mucus). Dextromethorphan is partly cleared by a liver enzyme called CYP3A4. In a lab dish, higenamine slowed that enzyme by about 21 percent, so in theory it could let dextromethorphan build up a bit and produce stronger effects like drowsiness, dizziness, or a foggy, lightheaded feeling.

I want to be honest, though: this has only been seen in test-tube studies, never confirmed in real people, so the actual risk looks small. What concerns me more is that higenamine itself can rev up your heart rate and blood pressure. If you take both regularly, just give your pharmacist or doctor a quick heads-up first.
