Fentanyl citrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Higenamine interacts with.

Theoretically, higenamine might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research shows that higenamine inhibits CYP2D6 enzymes. However, this effect has not been reported in humans.

Here's the honest picture: this concern is based only on lab (test tube) studies showing higenamine can block an enzyme called CYP2D6. It has not been shown to happen in real people. Fentanyl is mainly cleared by a different enzyme (CYP3A4), so CYP2D6 plays only a minor role for it. That makes a meaningful interaction here pretty unlikely on paper.

That said, fentanyl is a very potent opioid with little room for error. Anything that even slightly raises its levels could deepen its effects, think heavy drowsiness, confusion, or dangerously slow breathing. Higenamine itself can also stress your heart and blood pressure. Please don't pair these without talking to your doctor or pharmacist first.

Here's the gist for fentanyl. Your liver uses an enzyme called CYP3A4 to clear fentanyl out of your system. In lab dish studies, higenamine slowed that same enzyme down by about 21%. If that happened in a real person, fentanyl could hang around longer and build up, which means stronger effects than expected.

With a drug as powerful as fentanyl, that's worth taking seriously. Too much fentanyl can cause heavy drowsiness, confusion, very slow or shallow breathing, and dangerously low oxygen. On top of that, higenamine itself is a stimulant that can raise heart rate and blood pressure, so it's a rough mix. The evidence here is only from lab studies, not people, so we can't say it will definitely happen, but please clear higenamine with your doctor or pharmacist first.
